STUDENT MEDICAL HISTORY FORM (for registration)

If your student is in need of regular prescription oral medication during school hours, you may make a request for the school nurse to administer that medication. The following form MUST be completed by your primary caregiver and the instruction on the form must be followed PRIOR to the medication being brought on campus. STUDENTS ARE NOT ALLOWED TO BRING ANY MEDICATION, PRESCRIBED OR NOT, ON TO CAMPUS. Please call and discuss details with Nurse Pickett.
